Hi Emre,
The NBS manual provides numerous examples of design matrices and some tips on formulating a design matrix. The manual is available for download from this site (see the Downloads section).
The Help buttons in the GUI also provide numerous examples of design matrices for various statistical tests.
Finally, the FSL FAST website is another excellent resource for design matrices:
